Our drivers must be at least 25 years of age and capable of driving mini-vans and 8-12 passenger buses safely, while providing a pleasant experience for all passengers.

Position Overview:

Provides quality customer service while transporting customers in a safe and efficient manner, all while following program guidelines and in keeping with the goals, philosophy of service policy and objectives of the agency.

Essential Functions:

Core Requirements & Driving Skills

  • Maintains a valid PPC or CDL license and driving record in compliance with Catch-A-Ride’s BMV Record Policy. Reports any accidents/traffic violations immediately (even if incurred while off-duty).
  • Maintains a valid CPR/1st Aid certification.
  • Adheres to Physical & Drug Testing requirements.
  • Attends all required trainings provided by the Rural Transit Assistance Program (RTAP); Passenger Assistance, Defensive Driving, Emergency Evacuation, and Pre-Trip Inspections.
  • Follows all rules of the road, practices defensive driving and displays necessary driving skills in all situations.

Customer Service & Passenger Assistance

  • Assists all passengers as necessary from their origin to destination (without passing through any doorways); offers a helping hand to assist with safe boarding and disembarking and/or to carry bags when necessary. Ensures that all passengers wear seatbelts at all times and does NOT assist with child restraint devices.
  • Properly utilizes wheelchair ramp/lift and secures wheelchairs according to agency’s policies and procedures.
  • Builds a rapport with passengers and ensures that they have a good experience; a positive representative for Catch-A-Ride.
  • Submits Incident/Concern reports within 24 hours to report issues and/or clients who may be in need of additional services that LifeTime has to offer.

Dependability & Cooperation

  • Arrives on-time to begin shift, drives assigned vehicle on the assigned route, and stays on schedule throughout the day, reporting any issues to dispatch.
  • Attends all meetings.
  • Demonstrates flexibility through willingness to drive various routes, participates in schedule exchanges, and picks up shifts when feasible.
  • Delivers vehicles to vendors for maintenance/services as needed.

Administrative/General

  • Reviews daily manifest prior to shift, reports scheduling conflicts and/or changes to the manifest (No Shows, Cancellations, Incorrect Fares, etc.) throughout the day to dispatch.
  • Completes Daily Vehicle Inspection & Supply Log, reports any major concerns immediately, and submits at the end of each shift.
  • Completes any necessary 3rd party funder (Access2care, LCP, Modivcare, Verida, and WellTrans) paperwork, which includes obtaining signatures, and submits at the end of each day.
  • Collects cash fares as clients board, stores cash in a zipped bank bag Monday-Friday each week and makes one deposit by the following Monday. Signs and returns deposit slip (along with fuel receipts for the week) to the office promptly.
  • Communicates professionally with all team members. Ensures client confidentiality by using only client initials over the radio.
  • At the end of each day, checks vehicle for passenger belongings, fuels up, cleans vehicle (sweeps, removes trash, cleans windows, etc.), and ensures all systems are off, the vehicle is locked, and keys are stored in the lock box.
  • Submits timesheet at the end of each shift and leave requests as per agency policies and procedures.
  • Reads and responds to memos in a timely manner.
  • Completes Accident Reports and follows procedures in the event of an accident.

Community Health Worker

Lifetime Resources is seeking a compassionate, resourceful Community Health Worker to help individuals overcome barriers related to housing, food, transportation, healthcare, mental health, caregiving, and other social needs.

The Community Health Worker will:

  • Meet with individuals to assess their needs
  • Connect individuals with community resources and services
  • Conduct home visits and follow up to ensure needs are addressed
  • Build relationships with community organizations throughout our five-county service area
  • Maintain accurate referral, follow-up, and program data

Applicants must have strong communication, organization, and computer skills; the ability to work independently; a valid driver’s license; and reliable transportation. The Community Health Worker (CHW) will play a vital role in addressing Social Determinants of Health (SDOH) by providing direct support to individuals in need, connecting them with essential community resources, and conducting follow-ups to ensure their well-being. The CHW will work closely with the Aging and Disability Resource Center (ADRC) and community partners to facilitate personalized discussions, resource navigation, and data tracking to improve overall health outcomes.

This position is funded by the Rural Health Transformation Program.

Essential Functions:

Key Responsibilities

  • Conduct outreach and engage individuals identified through ADRC and partner referrals to assess SDOH-related needs such as housing, food, transportation, mental health, and caregiving.
  • Develop and implement an in-person assessment tool to identify challenges related to SDOH, including dementia or related diagnoses and social isolation.
  • Establish and maintain a tracking system for assessments, resource referrals, and follow-up actions to evaluate program effectiveness.
  • Provide individualized support by connecting individuals to healthcare, nutrition, social services, and wellness programs.
  • Conduct home visits as needed to facilitate resource navigation and provide emotional support.
  • Follow up with individuals to confirm resource utilization and determine if additional assistance is required.
  • Develop and complete post-assessment forms to document the resources provided and track follow-up actions.
  • Report monthly to the Director of Operations and HCC Director on grant statistics, challenges, achievements, and impact.
  • Research and maintain updated information on community resources, establishing partnerships with local organizations in the five-county region.
  • Represent the CHW program by introducing themselves to new community organizations and learning about their services and referral processes.
  • Participate in up to three yearly grantee meetings to share program insights and engage in grant evaluation and learning activities.
  • Attend and prepare for site visits with Interact for Health staff and collaborating organizations to review project details, budget, and anticipated outcomes.

Catch-A-Ride Driver Supervisor

 

Position Overview:

Responsible for driver supervision and assisting in operations, ensuring consistent provision of quality services in keeping with LifeTime Resources, Inc. goals and objectives, as well as requirements of Indiana Family and Social Services Administration, Indiana Department of Transportation, and other regulatory agencies. This is a Safety Sensitive position.

Essential Functions:

Management/Supervision

  •  Operates within budgetary guidelines; maintains cost control.
  •  Ensures quality services and continual improvement; meets required compliance standards; provides a vision for employees to attain; monitors the attainment of goals and objectives; provides technical assistance to employees as needed.
  • Communicates and coordinates with Drivers to develop system improvements.
  • Performs bi-monthly Driver observation assessments and shadow observations as necessary.
  • Ensures employees report incidents and concerns as necessary and adequately resolves reports in a timely manner.
  • Follows up on assigned Samsara Safety camera alerts for their drivers.
  • Monitors and provides feedback to drivers regarding their safety performance and responds to safety event alerts received through the Samsara monitoring system, as necessary.
  • Ensures adherence to the Accident and Moving Violation policy. Investigates all incident and accident reports and performs site investigations as needed. Coordinates and serves on the accident review committee and provides the Fleet Management Specialist with the necessary information to complete insurance claims.
  • Serves on Catch-A-Ride’s Service Review Committee to discuss any service modifications that may be necessary to ensure compliance with the Title VI plan.
  • Supervises Drivers; ensures the completion of personnel requirements including job descriptions, performance appraisals, disciplinary action, continuing education, and proficiency testing for compliance with federal, state, and local regulations, and service contracts. Maintains driver files.
  • Ensures employee adherence to policies and procedures, escalating to the Director when necessary.
  • Evaluates field operations on an ongoing basis; attains established goals.
  • Resolves field operation problems and addresses issues noted in quality assurance reviews.
  • Ensures professionalism in service and adherence to agency goals and philosophy.
  • Prepares Driver memos and incorporates them into Driver Manual where necessary.
  • Ensures vehicles are fully equipped with all paperwork (registration, insurance card, extra forms, etc.), vehicle postings (Title VI, Seat belts, Service Animals, No Smoking, Cameras in Use, Broker Contact Information, Spanish Translation, etc.) and supplies at all times.

 

Drug & Alcohol Program Management

  • Attends ITSA supervisor training for the FTA Drug and Alcohol program and provides assistance with the program as directed by the Drug and Alcohol Program Manager (DAPM).
  • Successfully completes the Designated Employer Representative (DER) and Reasonable Suspicion Training through RTAP bi-annually and reports suspicious activities and/or other issues as necessary.
  • Conducts quarterly random drug and/or alcohol testing in accordance with the Drug & Alcohol Policy

 

Maintaining Adequate Driver Staffing

  • Updates and distributes the Driver Manual on an annual basis.
  • Assesses system needs for drivers and as needed: interviews, completes new hire process, etc.
  • Ensures adequate supply of Driver shirts and orders as needed.
  • Ensures vehicle and wheelchair availability for demonstration, etc. during driver orientation.
  • During driver orientation covers introduction, CAR Policies, Compliance and RTAP training, general forms and procedures, tablet and EWS training, vehicle and safety review, pre-trip inspections, wheelchair securements, Outlook, Duo set up, etc. and is responsible for backing up the other driver supervisor as needed.
  • Prepares training materials, prepares HR packet(s), and gathers supplies in advance, ensures vehicle and wheelchair availability for demonstration, etc.
  • Ensures Driver Timesheets, Leave Requests and payroll reports are submitted and approved within a timely manner, ensuring all corrections are made, running biweekly payroll reports. Also, ensures Driver Availability is updated with all leave requests.
  • Adds new days to Easy Rides scheduling software and applies drivers to routes, finds drivers to cover open routes and maintains the Driver Availability tracking sheet.
  • Maintains CAR Driver Hours Report and ensures benchmarks are met, including the need to ensure that drivers do not exceed an average of 57 hours per pay period.
  • Maintains driver safety incentive program.

 

Other

  • Takes after-hours calls as assigned.
  • Assists with other program duties as needed.
  • Assists in fundraising and promotion of agency.
  • Serves as back up for the other Driver Supervisor, ensuring that all necessary tasks are accomplished in that person’s absence.

Home Care Manager

Advocacy & Service Coordination

  • Completes all tasks with a sense of urgency, effectively prioritizes to ensure timely and effective customer service.
  • Advocates for the individual by educating, encouraging, and empowering them to prolong their independence, recognizing situations that warrant additional coordination and support.
  • Completes person-centered assessments and develops person-centered care plans based on the individual’s needs, strengths, and preferences. Identifies options for service delivery including type of service(s), program eligibility and payment source(s) in keeping with agency philosophy, program requirements, and individual independence.
  • Ensures services authorized in plan of care are in place and addressing individual’s functional and environmental needs. Contacts each individual within required time frame to follow up on service start date. Continually monitors, reevaluates situations, addresses concerns, and updates care plans, as needed.
  • To meet temporary and/or ongoing needs that cannot be met through other formal and informal sources, develops creative solutions including recruitment of volunteers. Ensures continued identification of new resources to assist individuals and their caregivers.

Phone Options Counselor

  • Ensures safe and confidential handling of individuals’ information; provides excellent customer service.
  • Explains and assists in accessing non-agency options including entitlements; coordinates with informal and formal supports as needed.
  • Completes comprehensive, person-centered assessments when indicated and addresses both the functional and environmental aspects of the client situation to determine clients’ needs and preferences, while ensuring excellent customer service. Coordinates with formal and informal supports throughout the process, as needed.
  • Identifies options for service delivery including type of service(s), program eligibility and payment source(s) in keeping with agency philosophy and program requirements.
  • Advocates on individuals’ behalf.
  • Achieves established productivity standards in the areas of:
    • 100% of all activity completed is entered into the state approved software within 7 days
    • Effectively adds and monitors cases appropriate for the non-waiver and/or waiver waitlists
  • Appropriately identifies cases for case conferencing, particularly as it relates to handing off a case to another staff member and confers with supervisor as needed.
  • Performs volunteer recruitment and selection as needed to meet temporary and/or ongoing needs that cannot be met through other formal and informal sources.
  • Records all client related activity in case record; uses client database software program to enter all client data into system according to referral type; logs time spent on client activity in client database software.
  • Research and locates needed resources keeping in mind unique and creative person-centered solutions such as product or program information, shares requirements for various internal and external resources to assist with referral and/or application, as needed.
  • Keeps abreast of all home and community care rules and regulations, as relevant to referral type; ensures that services and activities adhere to program and state requirements.
  • Meets with all walk-ins on assigned days at the Dillsboro office to provide information, resources and assistance to access resources, as needed.
  • Educates clients, caregivers, and the community regarding the importance of donations.
  • Participates as needed in fundraising for the agency and represents the agency in the community.
  • Acquires and maintains SHIP certification, remains abreast of current Medicare issues; utilizing tools on Medicare websites to assist individuals.
  • Respects the client’s right to privacy.
  • Assigns Administrative Coordinator tasks as appropriate via Teams and monitors to ensure timely completion.
  • Refers individuals for Legal Aid, Ombudsman services, Senior Nutrition Activity Centers, housing, public transportation, community resources, guardianship services, APS, and other resources, as necessary, and completes follow up to ensure individual stability.
  • Distributes appropriate contact information and required agency and state forms to each individual.
  • Provides back up in the absence of On Call POC including completing referral calls.

 

Administrative/General

  •  Consistently and effectively utilizes position procedures; recommends changes when necessary.
  • Adheres to agency policies and procedures.
  • Keeps abreast of all home and community care rules and regulations.
  • Attends meetings and trainings as required and keeps abreast of issues and information necessary to perform work duties.
  • Understands, supports, and models the agency’s Mission Statement and Core Values
  • Engages in other related activities or special projects as required or assigned.
  • Promotes LifeTime/Catch-A-Ride and other services available at LifeTime.

Physical Effort: Sedentary Work: Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently; ability to climb stairs and independently access all customer homes.

Working Conditions: Office environment, regular travel in a personal vehicle; customer homes; possible exposure to bodily fluids; customer contact; occasional inclement weather.

Education, Experience and Training: A registered nurse—or—A Bachelor’s degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services—or—A Bachelor’s degree in any field with a minimum of two years full-time, direct service experience with the elderly or disabled (this experience includes assessment, care plan development, and monitoring)—or—A Master’s degree in Social Work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services may substitute for the required minimum of two full time direct services experience—or—An Associate’s degree in Nursing—or—An Associate’s degree in any field with a minimum of four year full-time, direct service experience with the elderly or disabled (this experience includes  assessment, care plan development, and monitoring). Must receive Case Management Certification from the state upon hire. Once certified, must attend 20 hours of approved training each year; must have a valid driver’s license.
